The Supervisor of Elections is required to receive the original voter registration application form containing the applicant's original signature. You may download, print, complete the Florida Voter Registration Application (DS-DE 38) English or Spanish, and then mail or hand-deliver to the Lake County Supervisor of Elections Office at 1898 E Burleigh Boulevard, Tavares, Florida 32778. Hours of operation are from Monday to Friday, 8:00 am-5:00 pm. Telephone: (352) 343-9734. To be eligible to vote for the August 23, 2022 primary election, a person must be registered no later than July 25, 2022 and for the November 8, 2022 general election, no later than October 11, 2022 which are the book closing deadlines.
